2003 VW Caravelle shreds/damages drive belt?

VW Caravelle with 85,000 km on odometer has had drive belt changed because the belt had failed.
Within six weeks the drive belt has failed again. All components appear to spin freely.

I expect that a tensioner is worn, allowing the belt to run out of alignment - check them all - there must be no side to side play.
