my 04 mondeo over heats after a short journey i leave it to cool then have to top my coolant up after what was remaining came flowing out when i removed the cap this keeps happening each time, when driving i lose power and when the engine is off a fan remains on for 30 minutes.

what is the cause and do you have the solution?
