My book (Golf 1998 - 2000) says...
"11.5 Lever off the cap from the top hinge pin and, with the aid of an assistant to support the door, remove the hinge pin from the top hinge. Also remove the two bolts from the lower hinge that secure the hinge to the door. Lift the door upwards and out to remove."
A picture shows the top hinge pin being removed with a small socket/ratchet spanner from below i.e it appears to unscrew downwards. Another picture shows the bottom hinge bolts being removed with what looks like a hex-key
