transit slave cylinder keeps bursting what could cause this to happen?

i recently fitted a clutch flywheel and slave cylinder (all brand new) on to an 02 plate 2ltrD fwd transit twice :( because the first time the cylinder burst and yet again the same thing has happend whatt could cause this problem?

how are you bleeding it because they should never be pressure bled that could cause the problem your experiencing
