my new clutch is still slipping why?its a vauxhall vectra r reg?

I recently had a new clutch fitted on my vectra r reg,,it still seems to be slipping

Did they skim the fly wheel when you had the new clutch fitted?
Why did the clutch fail?was it oil contaimination or something else?
my guess without seeing the car would be oil on the clutch,if that is the case who ever fitted the clutch has not done the job properly.
