opel zafira clutch?

i have had a clutch replaced 3 weeks ago, but it is now starting to slip again, what could be wrong?

take it back under warranty should last longer than 3 weeks unless you went to a cowboy, most clutches come with a 12 month warranty or 12,000 miles witch ever is sooner,
