how can i fix my transit van?

my van will not idle correctly and has white smoke coming from the exhaust. I have adjusted the tick-over screw, this does not solve the problem. I have changed the fuel filter and dont know what to do next. The engine is 2.5 diesel, any advice greatly recieved
