timing belt?

My timing belt slip off the camnshaft above a 1/8 of an inch,it is a new timing belt,what cause this,I presume it is a bad component but how can I tell which one.

i had this on my pug 205 diesel i replaced all the idlers tensioner and water pump , all were in contact with the belt, worn out idler pully bearings were causing exactly wat u describe good luck.
