With a 14 year old car it is likely that the rear pads, and possibly discs, need replacing. The adjustment is automatic and when the brakes are at the end of their life will adjust no further.

New rear pads and discs will cost about £60 from a good motor factors. You will need a caliper piston retracting tool (such as a cube) to wind the pistons back in.
