Parts for repair of Welbilt freezer chest lid hinges?

I have a Welbilt upright freezer chest, model # WCF-005W-2 (white in color) 3.8 cu.ft. The freezer has 2 hinges to affix the lid to the body of the chest in the rear to seal in the cold air. Looking to purchase the hinges.
