Remove the back wheel - and the cover in the centre of the hub. A single nut holds the brake drum, which houses the bearings. You have to pull the split pin, undo the nut and remove the drum. When you do the small outer bearing will fall out and you will see the larger inner bearing on the stub axle. That should slide off. You will need to replace the oil seal as well as the bearings. Inside the drum you will see the rings that the bearings run up against. They will need to be driven out and replaced with the new ones. Once you've done that, grease up the bearings and put it back together in reverse.
