How can I extend my concrete fence posts?

I need to extend my concrete fence posts by about 18 inches. I have seen somewhere on the web, metal sleeves which fit over the existing concrete post into which a piece on wooden post can be fitted. I am trying to fit a trellis to the top of the fence.

Why not get two mild steel plates the width of the post and about 200mm either side then drill holes through both 12mm and fit some galvanised iron bolts through - firstly the conc posts then the newly added post trellis.
