What does error 16 mean in car radio?

I have just installed a Dominator DM-3000 mp3/cd tuner. When i insert USB 'USB-DRV' is displayed but the 'error 16' comes up. the USB works fine on pc and in friends car.
I have read instructions but there is no faults thingy in it.
What am i doing wrong?

gave USb to teenage son & he reconfigured it - music was stored in separate files.
