My appologies but there so many of these mixer bath/shower fixtures that its nearly impossible to see how the fittings are related. Presuming the knob can be removed, if it is very old, (which it sounds) it may require heat to move it. Once you have introduced heat of a greater circumstance then you may be fully commited to removal as 'o' rings start to melt. Alternatively if the body is hexagonal and you can get a wrench on it then you can lightly tap the wrench near the jaws on the handle to gently release it.
No hexagonal body? then you will need stillsons and a rag to avoid taking the chrome off. If you heat it, then do so gently in stages with a blow torch lightly tapping and then finally use muscle power to release it(usually turning anti clockwise) No Joy? maybe a new shower mixer, dificult to say.. Good luck.. don't drown
