We have a 32" Samsung tv and, recently, on start up it goes to "ext 1", which is blank, and I have to use the "Source" button to change it to "ext 2" which takes me into Sky channels.

It used to go straight to Sky, but maybe we pressed a wrong button which made it do this?

use your tv remote control to set it on ext 2, when the tv is switched on. when you switch on tv next day press the sky button on the sky remote
