Check the small plastic pawl which sits on the teeth of the plastic gear (ratchet wheel). If that is split or broken it will not hold the wheel. Also the spring inside the gear wheel could be binding. Take it off and check spring. Replace if broken or distorted. Cover spring with plenty of fresh grease when replacing. Brake cable should not be the cause of this problem.
