Hi, check out your card for physical damage. If there is none take it back to the retailer and they should replace it under their warrantee policy.
Also, check that there is nothing on the contacts in the adapter.
If you think it might be a software you'll need to check this our with the vendor of your phone.
It may simply need to be formatted again, but do this with the phone and not on the PC.
