screen does not show writing at top bottom or sides I have tried adjusting position nothing seems to happen

there is a square digital box come with the signal on the screen which stays in middle this screen is for the signal indication, this sqre box hax gone on the top left corner it is like freeze there and nothing comes i have rtryied with dvd, cable, laptop, projector line still stays same please tel me what to do.
