whenever I turn on the hot water tap the water runs very hot for about 45 secs and then becomes tepid. What is wrong?

Having similar problem with ours , our boiler needs replacing as it is not viable to fix, too costly. boiler 15 years old,
much more efficient condensing combi boilers available.
Going for Worcester Bosch.
Sorry for bad news,
Ask one or two heating engineers.
