check for broken arm in the drive wheel.(can be on either end) as its only cast aluminium
check the drum bearings are not worn/collapsed push the inner drum upwards at the 12 o clock position excessive wobble bearing have gone
either of the above can cause the drive belt to come off, when the pulley wheel runs out of true alignment
