CH pressure too high?

My girlfriend recently had issue in that the CH pressure dropped to zero so no hot water etc. She 're-pressurized' the system & now it's constantly over 3 with water overflowing outside & from one of the radiators (from cap opposite to bleed valve!). We drained this radiator & re-filled (without re-pressurizing) & the pressure dropped to 2 but has now crept back up to over 3 again. Any ideas?

Thanks to you both for your answers - I had a little 'fiddle' with the tap for the filling loop & the pressure has now dropped to 1. Either it wasn't off totally (altho' it felt like it) or there was a piece of grit lodged in the valve.

Fill the system when cold (the boiler not the weather) then run the system untill it is hot. If the pressure rises past 2.5 or 3 bar or the pressure relief activates, then the pressure vessel will need replacing.
